The following notice has been sent to you because you have indicated an interest in, or been involved with the Clean Water Branch, Polluted Runoff Control Program grant, in the past. This email is to let potential applicants know that our FY10 Request for Proposals is open. Proposals are due on Thursday, June 24, 2010. REQUEST FOR PROPOSALS

No. CWB-PRC 10-02

Pursuant to Chapter 103D, HRS, the Department of Health’s Clean Water Branch is soliciting Request for Proposals (RFP) No. CWB-PRC 10-02 for polluted runoff control projects for Fiscal Year 2010-2011 Section 319(h) grant funding. This is a reimbursement grant. At a minimum, grant funds must be matched 100% with match funding or in-kind contributions from non-federal sources and must be documented with timesheets, volunteer sign-in sheets, receipts, etc. as determined by the Department of Health. Specific criteria for projects are listed in the RFP.


One (1) hard copy marked as the “original”, unbound (e.g., not stapled) and printed one-sided; five (5) additional hard copies, bound (e.g., stapled) and printed double-sided; and one (1) digital copy of the proposal on compact disc must be submitted. Offerors must submit proposals in the format specified in the RFP in order to qualify for award.

Proposals must be received and time stamped by the Procurement Officer (whether mailed or hand delivered) no later than 3:00 p.m., HST, on Thursday, June 24, 2010, at the Department of Health, Clean Water Branch, 919 Ala Moana Boulevard, Room 301, Honolulu, Hawaii 96814-4920. Proposals received after the deadline set for receipt of proposals will not be accepted. Faxed or e-mailed proposals will not be accepted or considered. The Department of Health reserves the right to cancel this RFP and to reject any and all proposals in whole or in part when it is determined to be in the best interest of the State of Hawaii.
